How Vinca Alkaloids Damage Peripheral Nerves
Vinca alkaloids stop cancer cells from dividing by blocking structures inside cells called microtubules. Curcumin: The Most Directly Studied Plant Compound
Curcumin is the primary polyphenol in turmeric (Curcuma longa). It has anti-inflammatory and antioxidant properties and has been studied in various nerve conditions. A double-blind, randomized clinical trial published in PMC enrolled 70 adult cancer patients with established platinum-induced CIPN. Participants received either curcumin at 40 mg twice daily or a placebo for four weeks. The curcumin group showed a significant reduction in sensory CIPN symptoms compared to the placebo group. The researchers suggested that curcumin’s antioxidant and anti-inflammatory properties may protect nerve fibers from further damage.
Standard curcumin from turmeric powder absorbs poorly through the gut. The trial used a specific formulation designed to improve absorption, and results may not apply to low-bioavailability products. Bioavailability-enhanced extracts are the forms used in clinical research. When choosing a product, look for one that states its standardization and absorption method clearly. Some products use the BCM-95 enhanced extract, a form studied for better absorption than standard curcumin powder.

Alpha-Lipoic Acid: An Antioxidant With Mixed Trial Results
Alpha-lipoic acid (ALA) is found in small amounts in foods such as spinach, broccoli, and yeast. It acts as an antioxidant in both water-soluble and fat-soluble parts of cells, so it can reach many tissues including nerve cells. ALA supplements have a known safety record at doses of 600 mg to 1800 mg per day, based on a systematic safety review in PMC.
Results in CIPN specifically are mixed. A randomized, double-blind study in PMC gave cancer patients receiving cisplatin or oxaliplatin chemotherapy either 600 mg ALA or placebo three times daily for 24 weeks. There were no statistically significant differences in worst pain, average pain, or pain interference between the two groups. Only 29% of people finished the full study, which limits what we can conclude. Some smaller studies using 600 mg once daily during chemotherapy reported improvements in sensory neuropathy, but larger studies haven’t confirmed this.
When to take ALA supplements relative to chemotherapy infusions is not yet settled. Some oncologists worry about antioxidant supplements during active treatment. Discuss timing specifically with your care team before starting ALA.
Omega-3 Fatty Acids: Anti-Inflammatory and Preliminary

Vitamin E: A Preliminary Protective Signal
Vitamin E is a fat-soluble antioxidant that protects cell membranes from oxidative damage. Nerve cell membranes have a lot of fat, so they can be harmed by oxidative stress during chemotherapy. The same PMC systematic review that highlighted omega-3 also noted that vitamin E may help prevent CIPN based on early research. The evidence is early-stage, and a dose for CIPN prevention or treatment has not been confirmed in large trials.
High-dose vitamin E can interfere with blood clotting and may interact with blood thinner medications. Researchers debate whether antioxidant supplements during chemotherapy could reduce how well treatment works. Talk to your oncologist about vitamin E, especially if you’re still getting chemotherapy rather than in the recovery phase.
Exercise and Practical Safety During Neuropathy
Physical exercise is one of the few non-pharmacological approaches that the ASCO CIPN guideline supports. The guideline says regular activity like walking, balance training, strength exercises, and stretching may help manage CIPN and improve how well you function. Exercise helps blood flow to the nerves and may reduce swelling in nerve tissue over time.
